Change Your Health Plan
If you are already enrolled in a Health Plan and would to change to a different Health Plan, you must meet one of the following requirements:
- It has been less than 90 days since your initial enrollment, or
- You tried to get help from your PCP, but continue to have problems finding the care you need.
If you do not meet one of the requirements above and still want to change your Health Plan, you may change your Health Plan during open enrollment at the end of each year.Online
If you want to change a Health Plan, the member portal will walk you through the change process step-by-step. When you change a Health Plan, be sure to choose a primary care physician for each person in your family or the Health Plan will choose one for you.

If you know which health plan you want to change to, fill out the health plan change form included within the welcome packet and mail it in the envelope provided.
